New Patient Appointment Guide
To schedule your first appointment, we will need the following:
Your first and last name as it appears on your Medicare Card
Date of Birth
Best Contact Number
Your referring GP’s: name, phone number and practise name
Please have these ready when you call us. We will ask you some questions and the reason for your appointment in order to triage and allocate the appropriate amount of time for your visit. Alternatively, email us a copy of your GP referral to admin@drnipu.com.au.
You will be given the date and time of your appointment and information confirming your appointment will be sent to you via SMS and email.
What to bring to your appointment:
Medicare Card
Private Health Fund details
A valid referral from your GP
Relevant Breast Imaging (mammogram, ultrasound, MRI or other scans)
Biopsy reports including previous surgery
Details of any previous surgery
Details of other Medical Conditions, specialist details
Details of current Medications
A list of questions you may have
If you are unable to speak English and require assistance with understanding or expressing yourself, kindly bring a relative or friend who can translate for you. If you need a translator, please inform us 2 t0 3 days before your appointment.
Need to cancel an appointment?
Please call the office during business hours and provide at least one day’s notice so we can reschedule your appointment.
Please note that we strive to be as punctual as possible; however, occasionally, some patients may require more attention than anticipated.
